AI scribes for NPs and PAs in small independent practices

Nurse practitioners and physician assistants carry the same documentation load as physicians in most independent practices — often more, because APPs frequently handle the acute and follow-up volume that fills clinic days. Every reason a physician reaches for an AI scribe applies at least as strongly to an APP.

For solo NP-led primary care, urgent care, or specialty clinics, the scribe fit is identical to the physician case: cloud EHR, self-serve procurement, month-to-month, BAA signed inline.

In states or systems that require physician co-signature on APP notes, the scribe should produce a note that the supervising physician can review and co-sign without re-writing. Nabla and Twofold surface a clear draft-and-cosign workflow. Freed and Heidi produce clean notes but leave co-signature routing to the EHR. Suki and Abridge are physician-first and expect one clinician per encounter.

State law defines when co-signature is required. The scribe does not track scope-of-practice; the clinician does. Do not rely on the vendor to enforce scope-of-practice rules.

APP encounters bill under the same E/M codes as physician encounters (with state-specific modifiers and incident-to considerations). Coding-assist behaves identically. Abridge has the strongest coding-assist output; Twofold, Freed, and Nabla are close behind.

For an NP-led or PA-led small practice on a cloud EHR, Twofold Health is the most-cited starting point in our 2026 evaluation. Freed is a strong alternative when the practice has one or two clinicians and wants the shortest onboarding, and Nabla is the better fit when the practice requires structured co-signature routing and is on Athena or Epic.
