SOAP note

A four-part clinical note: Subjective, Objective, Assessment, Plan.

SOAP is the dominant structure for a clinical progress note. Subjective is what the patient reports; Objective is what the clinician observes and measures; Assessment is the clinical interpretation; Plan is what the clinician will do about it. Most AI scribes default to SOAP output but can be templated to DAP, APSO, or specialty-specific formats.
