Fabricated content in an AI-generated note that was not in the encounter.

A hallucination is content the AI scribe produces that was never said or observed in the visit — a phantom medication, an imagined ROS finding, a fabricated exam result. Hallucination rate is the single most important note-quality metric; every note must be reviewed and attested by the clinician before it enters the chart.
