Electronic Health Record — the software of record for a patient chart.

An Electronic Health Record (EHR) is the digital patient chart. Common EHRs used by small clinics include Athena, eClinicalWorks, DrChrono, Elation, Practice Fusion, Kareo (Tebra), Epic (via community-connect programs), and NextGen. AI scribes 'integrate' with an EHR by writing the generated note back into the chart, either natively, via SMART on FHIR, or by copy-paste.
