DAP note
Data, Assessment, Plan — the dominant format in mental-health documentation.
A DAP note has three sections — Data (what happened and was said), Assessment (clinical interpretation), and Plan. It is the dominant format for psychotherapy and counseling documentation. Most AI scribes support DAP alongside SOAP.
