Evaluation and Management (E/M) codes describe outpatient visit complexity and drive most primary-care revenue. Since the 2021 CMS rule change, E/M levels are set by medical decision-making or total time. Some AI scribes suggest an E/M level from the note; final selection is the clinician's responsibility.
