Chief complaint

The patient's stated reason for the visit, in their own words where possible.

The chief complaint is the one-line reason the patient came in, ideally captured close to the patient's own words. AI scribes should preserve patient phrasing rather than paraphrasing into clinical jargon at the top of the note.
